Welcome to ImmigrationPhysican.com.  A. Van Caneghem  MD, FACP is a Board Certified internist whose office is designated by the United States Citizenship and Immigration Service (USCIS) to fulfill the medical requirements of applicants to U.S. permanent residency (Green Cards).

A medical exam is done and all reports are issued in 2 days.  Expedited service is available at an additional cost.

Directions:
Dr. Van Caneghem's office is located in Manhattan, at 408 West 57th Street, between 9th and 10th  avenues, easily accessible by buses and subways.

Take M11, M31 or M57 buses to within a few steps from the office. Alternatively,  all buses or subways that stop at Columbus Circle, Lincoln Center or Carnagie Hall will leave you within walking distance from the office.

 Look for the sign in front of the building that says "The Geneva Building"

Other Services:
The office is equipped to administer vaccines for travelers and  is New York State designated Yellow Fever vaccination center. 


Changes since 2009

  • HIV test is not required.
  • HPV vaccine not required.
  • Zoster vaccine not required.
  • The IGRA blood test for TB can be done instead of the skin test.  It is considered superior to the old test and needs only one visit.  Please ask Dr, Cangehem for details.

INS Medical Requirements:
Medical requirements for the purpose of obtaining a U.S. permanent residency (Green Card) are as follows:
  1. Physical examination*
  2. Tuberculosis Skin Test*
  3. Blood test for Syphilis*
  4. Completion of all forms*

Instructions: 

  1. Obtain the form I-693 (Medical Examination of Aliens Seeking Adjustment of Status).  Do not fill out the form.
  2. Call us to make an appointment for your examination.
  3. Bring your vaccine records when you come in.

What to expect:
You will need a total of 2 visits.  
On your first visit you will receive the medical examination,  the Tuberculosis and the Syphilis blood test. 

You will also receive immunization shots if necessary.

When you come back, on your second visit, the doctor will have the results of all the tests.
